Why Us
A health care directive speaks for you when you cannot. We write your choices into the California form and return it to you so you can talk it over with the people you name.
Questions
The law says the directive must be signed and dated, with either two qualified witnesses or acknowledgment before a notary. The agent and the signer's health care provider cannot be witnesses, and one witness must not be related or an heir.
The law adds a special requirement for residents of a skilled nursing facility. A patient advocate or ombudsman must act as a witness.
No, a HIPAA authorization lets people you name receive your medical information. A directive names an agent and records wishes. Many people sign both, but which ones you need is your decision. We prepare what you direct.
Yes, you decide when to update it. We can prepare a new directive from your updated choices. Please make sure your agent and doctors have the current copy, since an old copy can cause confusion.
No, we cannot give medical or legal advice. We type your choices into the document in your words. A doctor can explain medical options, and an attorney can advise on legal questions.
